Safe Skies offers State of Florida Live Scan services to individuals from anywhere in the World who are seeking a Professional license or Florida background check. The process is simple, fast and convenient.
Client’s must be fingerprinted by a Law Enforcment or certified Fingerprint agency on 2 (two) FD-258 FBI fingerprint cards using black ink. All demograph information on the cards must be completed using black ink.
The fingerprint cards, along with a completed Safe Sky Client Registration form, are then sealed in a FedEx or UPS envelope and the fingerprint collector must initial the seal. Client may take the FedEx or UPS envelope to the shipping location. Shipping by regular US mail is not acceptable.
The envelope is sent to: Safe Sky, Inc. 608 SE 6th Street, Suite 9, Fort Lauderdale, Florida 33301.
Client emails Safe Sky at info@safesky.us with the FedEx or UPS tracking number, your name and contact telephone number.
If you do not have have two FD-258 fingerprint cards, we can mail them to you.
Safe Sky will send you an electronic invoice and you may pay securely on line for the services you select.
Safe Sky is not responsible for fingerprints rejected due an incorrect ORI or poor quality. Please verify the ORI before you mail the cards. We will review quality of prints before submission.
Fee’s for service are:
Electronic submission to Florida Bureau of Professional Standards for ORI’s beginning with FL are $79.00.
Electronic submission to Florida Medical Board for license application ORI’s beginning with ED are $96.00.
3 (three) FD-258 fingerprint cards mailed to you is just $12.00.
3 (three) FD-258 fingerprint cards sent to you via FedEx standard 2 day is $25.00.
